Medical service agreement
Party A: the first party, the first, the second and the third.
Party B: the first party, the first, the second and the third.
Medical Technology Development Co., Ltd. is a provider of medical information for members. Consultation The professional agreement of medical service intermediary, the two sides reach the following agreement on service items:
1. from the date of signing this agreement, Party B (or Party B's designated person of service) will become a member of the health club of Party A for a period of one year.
2. Party B shall pay the membership fee at one time.

The subjects are the three groups of people.
3. Party A shall provide Party B with the following services within the time limit:
1) conduct a comprehensive Western medical check-up, psychological assessment and health history survey for Party B (see the body hospital and the project details in Annex 1).
2) according to the results of the examination, the experts shall assess the health of Party B and make health care.

3) establish a detailed, comprehensive and dynamic "health record" for Party B.
4) instruct Party B to carry out "sub-health" conditioning.
5) provide health care club members with a full range of guide services: emergency, outpatient service, consultation, hospitalization and operation (hospital expenses incurred, and the list of special hospitals is detailed in Annex 2).
6) arrange health consultation and health tracking for health care providers.
7) timely lectures on health knowledge and health care experience salon.
8) provide Party B with timely medical information and health care knowledge.
4. the two sides should consult in a sincere and friendly manner if there is any dispute in the process of cooperation.
If no settlement can be reached through negotiation, either party shall have the right to submit the dispute to the economic and Trade Arbitration Commission of the people's Republic of China in accordance with the arbitration rules of the Arbitration Commission.
The arbitral award is final and binding upon both parties.
